Insurance Information for Retirees

Notice :

Effective September 30, 2009, H1N1 flu vaccine and standard influenza immunization shots administered at a pharmacy are a covered benefit, subject to co-payments and deductibles.
Instructions:
Pay for the immunization administered at the pharmacy and get a receipt (with the date, the cost, the name of the vaccine, the plan member who received the vaccine and the name of the provider who administered it) indicating you paid for the immunization. Sign and date the form and send it to the address on the form. If you have EPO or HMO: For the EPO (UnitedHealthcare), HMO (Humana) and Medical Home HMO (Vantage) health plans go to www.groupbenefits.org for links to your respective health plan. Effective July 1, the Catalyst Rx mail service pharmacy provider for PPO, EPO and HMO plan members changed from Walgreens Mail Service to Immediate Pharmaceutical Services (IPS). One-Time Enrollment To begin receiving prescriptions by mail, you need to enroll with IPS-by mail, Internet or telephone. Mail: Complete a Registration and Prescription Order Form and mail it to IPS. This form establishes your health, allergy and plan information with IPS. Internet: Access the Catalyst Rx website (www.catalystrx.com). Using the OGB plan member’s Social Security number and birth date, log in to the member website and follow the instructions for New Enrollment on the Mail Service page to complete and submit an online enrollment form. Upon receipt, your enrollment will be processed within two business days. Phone: Call Catalyst Rx toll-free at 1- 866-358-9530. Enrollment is available Monday through Friday from 7 a.m. till 9 p.m. (CST) and Saturday from 8 a.m. until 4 p.m. (CST). Remember to have your health and allergy information as well as your prescription member ID card available prior to placing your call.   Eligibility:
  • You may enroll in the Office of Group Benefits (OGB) program if you are a full-time employee.

  • New Employees:
    Health coverage becomes effective 30 days after the 1st of the following month.

  • Late Applicants:
    I f you have a qualifying event sign up by the 14th of any month and Insurance is effective the following  month. Insurance premiums are deducted from your paycheck the prior month of the effective date.

For Retirees

The amount of your premium depends on the number of years you’ve had insurance cover-age. If you were enrolled with OGB (formally known as the State Employees Group Benefits Program) prior to November 14, 2001, then you are vested with OGB and your premium will be subsidized 75% by the State. If you were not vested by this time, then the percentage of State Subsidy is reduced depending on the number of years you carried health insurance prior to retirement.

When you retire you may keep the health insurance through OGB. As a courtesy to the retiree, TRSL and LSERS will deduct your Health & State offered Life Insurance from your retirement check; however, it takes several months for this to occur.   • HMO  Health Maintenance Organization

    For those state employees who prefer to receive their care through a health maintenance organization, Group Benefits offers LPPS Employees coverage through Humana. You may contact Humana  directly at 1-800-564-9159 , or visit their web site.

Changes

We have open enrollment for health insurance in October. You are allowed to change carriers during this time only.

Marriage: Plan member must complete an enrollment/change form to increase health and/or dependent life within 30 days of the marriage. A marriage license is required.

Newborns: Plan member must complete an enrollment/change form within 30 days from date of birth. A temporary birth certificate is required.

Cancellation of Coverage: an enrollment/change form must be completed.

Please notify of address changes.

Please include Employee ID # and Social Security # on all correspondence.
